Immuno-allergic interstitial nephritis related to fluindione: first biopsy proven cases.

چکیده
Case 1. A 72-year-old man was admitted to our unit for acute renal failure (serum creatinine 353 mmol/l vs 107mmol/l at baseline). He had started fluindione for atrial fibrillation five weeks before. Clinical examination was normal. There was no eosinophilia. Urine protein/creatinine ratio was 25mg/mmol, containing low molecular weight protein without albumin. There was no haematuria or pyuria. Renal ultrasonography was normal. Fluindione was stopped at admission. Transjugular renal biopsy disclosed interstitial lymphocytic infiltration with severe tubulitis. Immunofluorescent studies revealed no immune deposits. The renal function returned to baseline level 2 weeks after fluindione was stopped, without corticosteroid therapy.

Fluindione-induced immuno-allergic interstitial nephritis
Drug-induced acute interstitial nephritis (AIN) is an established cause of acute kidney injury (AKI). Antibiotics and non-steroidal anti-inflammatory drugs (NSAIDs) are the most frequent offending drugs [1]. Only a few vitamin K antagonist-induced AIN cases have been reported. Some publications describe AIN associated with fluindione (Previscan R ©), an anticoagulant of the antivitamin K family...
متن کاملAcute and chronic nephropathy induced by fluindione must be addressed.
BACKGROUND Among the vitamin K antagonists (VKA), indanedione-derived VKA is suspected to induce an immunoallergic risk. One indanedione-derived VKA, fluindione, is still being used in France. The aim of this study was to evaluate the contribution of VKA to acute and chronic nephritis. METHODS Twenty-four cases of biopsy proven acute interstitial nephritis (AIN) were retrospectively selected,...
متن کاملReversible Fluindione-Induced Chronic Interstitial Nephritis
Fluindione is well known to induce acute drug-induced interstitial nephritis (IN). Most cases occurred soon after the onset of treatment. We report a unique case of severe subacute fluindione-induced IN diagnosed 2 years after the treatment was begun. Renal function dramatically improved after fluindione withdrawal and steroid therapy.
متن کاملAllergic Interstitial Nephritis Manifesting as a Striated Nephrogram
Allergic interstitial nephritis (AIN) is an underdiagnosed cause of acute kidney injury (AKI). Guidelines suggest that AIN should be suspected in a patient who presents with an elevated serum creatinine and a urinalysis that shows white cells, white cell casts, or eosinophiluria. Drug-induced AIN is suspected if AKI is temporally related to the initiation of a new drug. However, patients with b...
متن کاملAzathioprine-induced interstitial nephritis in an anti-neutrophil cytoplasmic antibody (ANCA) myeloperoxidase (MPO) vasculitis patient
Azathioprine (AZA) is used in a wide array of autoimmune diseases, still corresponding to the mainstay maintenance therapy in anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitides. Although generally well-tolerated, several side effects are recognized. We report the case of a 50-yearold Caucasian man with kidney-limited ANCA myeloperoxidase (MPO) vasculitis who presented with gene...
